A Healthy Routine


Seeing the same doctor when you're sick is smart, say researchers at the University of Washington School of Medicine. In a study of 42, 700 people, those who regularly visited a particular health care centre were 16 per cent more likely to get flu shots and 19 per cent more likely to have mammograms than those with no regular site of care. Those who saw the same doctor were even likely to do so.

Why? Patients may be more willing to trust and listen to familiar doctor's recommendations. And doctor who know their patients are likely more aware of what preventive measures are needed.

Dress Down, Slim Down

They gave 53 people who work in in banks, nursing homes, schools and bookshops a pedometer to track their activity. When the workers wore jeans or khakis, cotton shirts and comfortable shoes, they took about 500 more steps and burned more kilojoules (kJ) than on dressier days.

Most of us add up to one kilo a year; dressing down daily could prevent this weight gain, says lead researcher Katie Zahour, an exercise physiology graduate student.

In transit On a plane, ask for a window seat in the front, away from engines and, if you fly a lot, consider buying special noise-cancelling headphones. Bring a C-shaped pillow for head support or, if there are extra pillows on board, make one by wrapping two pillows in a blanket. In the car (for passengers only!), block beadlights with a sleeping mask.

In a hotel Pack calming aromatheraphy oils or scented candles and a soothing white-noise or ambient music CD. Ask for quiet room away from the street and lifts, and set the temperatures between 20 and 22 degrees.

At a friend's house Take a warm bath to induce slumber. If you get up to use the toilet, don't hit the switch, as light slashes melatonin, waking you. You can also pack a dimmable nightlight.

Pneumothorax, Tension and Traumatic

Background

A pneumothorax refers to a collection of gas in the pleural space resulting in collapse of the lung on the affected side. A tension pneumothorax is a life-threatening condition caused by air within the pleural space that is under pressure; displacing mediastinal structures and compromising cardiopulmonary function. A traumatic pneumothorax results from blunt or penetrating injury that disrupts the parietal or visceral pleura. Mechanisms include injuries secondary to medical or surgical procedures.

Pathophysiology

A tension pneumothorax results from any lung parenchymal or bronchial injury that acts as a one-way valve and allows free air to move into an intact pleural space but prevents the free exit of that air. In addition to this mechanism, the positive pressure used with mechanical ventilation therapy can cause air trapping.

As pressure within the intrapleural space increases, the heart and mediastinal structures are pushed to the contralateral side. The mediastinum impinges on and compresses the contralateral lung.

Hypoxia results as the collapsed lung on the affected side and the compressed lung on the contralateral side compromise effective gas exchange. This hypoxia and decreased venous return caused by compression of the relatively thin walls of the atria impair cardiac function. The decrease in cardiac output results in hypotension and, ultimately, in hemodynamic collapse and death to the patient, if untreated.

What is Pnemothorax?

Background

Pneumothorax is defined as the presence of air or gas in the pleural cavity. Primary spontaneous pneumothoraces (PSPs) occur in people without underlying lung disease or trauma to the thorax. Many patients whose condition is labeled as primary spontaneous pneumothorax have an unrecognized lung disease. Secondary spontaneous pneumothoraces occur in people with underlying parenchymal lung disease. Traumatic pneumothoraces result from injury, often secondary to medical intervention (ie, iatrogenic pneumothorax). When air is trapped in the pleural cavity, resulting in positive pressure, a tension pneumothorax develops.

Pathophysiology

Primary spontaneous pneumothoraces result from apical pleural blebs lying under the visceral pleura. Primary spontaneous pneumothoraces are typically observed in tall young people without parenchymal lung disease because of increased shear forces in the apex. While patients do not have overt parenchymal disease, they commonly are smokers. More than 90% of patients with primary spontaneous pneumothorax are smokers. The relative risk of primary spontaneous pneumothorax increases as the number of cigarettes smoked per day increases. This incremental risk with increasing number of cigarettes smoked per day is much more pronounced in female smokers. Eighty percent of patients have emphysemalike changes on CT scan, particularly in upper long zones.

The body of evidence that genetic factors may be important in the pathogenesis of many cases of primary spontaneous pneumothorax is growing. Familial clustering of this condition has been reported. Several genetic disorders have been linked to primary spontaneous pneumothorax. Marfan syndrome, homocystinuria, and Birt-Hogg-Dube syndrome are among such disorders. Birt-Hogg-Dube syndrome is an autosomal dominant disorder that is characterized by benign skin tumors (hair follicle hamartomas), renal and colon cancer, and spontaneous pneumothorax. The spontaneous pneumothorax occurs in about 22% of patients with this syndrome. The gene responsible for this syndrome has been identified and is located on chromosome 17. Genetic testing is now available for Birt-Hogg-Dube syndrome.

Secondary spontaneous pneumothoraces (SSP) occur in the presence of lung disease, primarily in the presence of chronic obstructive pulmonary disease (COPD). Other diseases that may be present when secondary spontaneous pneumothoraces occur include tuberculosis, sarcoidosis, cystic fibrosis, malignancy, and idiopathic pulmonary fibrosis.

Pneumocystis jiroveci pneumonia (previously known as Pneumocystis carinii pneumonia [PCP]) was a common cause of secondary spontaneous pneumothorax in patients with AIDS during the last decade. With the advent of highly active antiretroviral therapy (HAART) and widespread use of trimethoprim-sulfamethoxazole prophylaxis, the incidence of PCP and associated SSP has significantly declined. This disease is now primarily seen in patients who are noncompliant with HIV therapy or trimethoprim-sulfamethoxazole prophylaxis or those taking inhaled pentamidine for PCP prophylaxis. PCP in other immunocompromised patients is seen only when trimethoprim-sulfamethoxazole prophylaxis is withdrawn prematurely. For practical purposes, if the immunocompromised patient has been taking trimethoprim-sulfamethoxazole prophylaxis reliably, PCP is reasonably excluded from the differential diagnoses.

Traumatic pneumothoraces can result from both penetrating and nonpenetrating lung injuries. Complications include hemopneumothorax and bronchopleural fistula. Traumatic pneumothoraces can create a 1-way valve in the pleural space, only letting in air without escape, and can lead to a tension pneumothorax.

Iatrogenic pneumothorax is a complication of medical or surgical procedures. It most commonly results from transthoracic needle aspiration. Other procedures commonly causing iatrogenic pneumothorax are therapeutic thoracentesis, pleural biopsy, central venous catheter insertion, transbronchial biopsy, positive pressure mechanical ventilation, and inadvertent intubation of the right mainstem bronchus. Therapeutic thoracentesis is complicated by pneumothorax 30% of the time when performed by inexperienced operators in contrast to only 4% of the time when performed by experienced clinicians. The routine use of ultrasonography during diagnostic thoracentesis is associated with lower rates of pneumothorax (4.9% vs 10.3%) and need for tube thoracostomy (0.7% vs 4.1%). Similarly, in patients who are mechanically ventilated, thoracentesis guided by bedside ultrasonography without radiology support results in a relatively lower rate of pneumothorax.

Tension pneumothorax typically occurs in the intensive care setting in patients who are ventilated. With air trapping in the pleural space, positive pressure rises. This pressure compresses the mediastinum, decreasing venous return to the heart and reducing cardiac output. In addition, owing to ipsilateral lung collapse and contralateral lung compression, gas exchange is compromised, leading to hypoxemia.

Call your health professional to have your child evaluated for scoliosis if:

  • You observe a curve in your child's spine.
  • You notice that something about your child's posture looks unusual, such as ribs that stick out, one shoulder that is higher than the other, one hip that is higher than the other, or an uneven waistline.
  • You observe that your child's clothes don't fit properly or that his or her hems don't hang evenly.
  • A school screening program recommends that your child see a doctor.

Watchful Waiting

If you suspect that your child has a spinal curve, ask a health professional to look at it, either through a school screening program or during a doctor visit. Early detection could lead to early treatment and could prevent a curve from getting worse.

If the results of a school screening program suggest that your child may have a spinal curve, follow up with your doctor. Most curves that are detected through school screening programs are normal variations in the spine or mild scoliosis, and they usually require only regular observation.

The following health professionals could identify and monitor scoliosis:

A doctor who specializes in surgery of the bones (orthopedic surgeon) may be consulted if the person has a large curve. The orthopedic surgeon will evaluate the curve and may recommend bracing or surgery.

Scoliosis - Cause

In about 80% of cases, the cause of scoliosis is not known.1 This is called idiopathic scoliosis. It develops mostly in children and teens and appears to be related to several factors, including genetics, as it often runs in families.

There are two types of scoliosis: nonstructural and structural.

Nonstructural scoliosis involves a curve in the spine, without rotation, that is reversible because it is caused by an underlying condition such as:

  • Pain or a muscle spasm.
  • An inflammatory condition, such as appendicitis.
  • A difference in leg length.

Structural scoliosis involves a curve in the spine, with rotation, that is irreversible and is usually caused by an unknown factor (idiopathic) or a disease or condition such as:

  • Disorders that were present at birth (congenital), such as spina bifida, in which the spinal canal does not close properly; or a disorder that affects the formation of bones. Between 5% and 7% of scoliosis cases are caused by congenital abnormalities of the spine, which cause curves in the spine that are more rigid than those caused by idiopathic scoliosis. These curves often get worse as the child grows, especially during the teen years.1
  • Nerve or muscle disorders, such as cerebral palsy, Marfan syndrome, or muscular dystrophy.
  • Injuries.
  • Infections.
  • Tumors.

In adults, scoliosis may result from changes in the spine due to aging (degenerative changes). These degenerative changes may be caused by osteoarthritis or osteoporosis.

What is scoliosis?

Scoliosis is a problem with the curve in your spine. Some curves in your spine are normal. But a few people have spines that make a large curve from side to side in the shape of the letter “S” or the letter “C.” If this curve is severe, it can cause pain and make breathing difficult.

The good news is that most cases of scoliosis are mild. If found early, they can usually be prevented from getting worse.

What causes scoliosis?

In most cases, the cause of scoliosis is not known. Scoliosis usually starts between the ages of 8 and 10.1 Scoliosis that is severe enough to need treatment is most common in girls.2

A curve in the spine may get worse as your child grows, so it is important to find any problem early.

What are the symptoms?

Scoliosis most often causes no symptoms in your child until the spinal curve becomes large. You might notice these early signs:

  • Your child has one shoulder or hip that looks higher than the other.
  • Your child’s head does not look centered over the body.
  • Your child has one shoulder blade that sticks out more than the other.
  • Your child’s waistline is flat on one side, or the ribs look higher on one side when your child bends forward at the waist.

In adults, scoliosis may cause back pain and trouble breathing.

How is scoliosis diagnosed?

The doctor will check to see if your child’s back or ribs are even. If the doctor finds that one side is higher than the other, your child may need an X-ray so the spinal curve can be measured.

Scoliosis is most serious in young children who are still growing. A curve in the spine may get worse as your child grows. So screening your child for scoliosis is important so that any curve in the spine can be found early and watched closely.

How is it treated?

Mild cases of scoliosis usually do not need treatment. Your doctor will check the curve of your child’s spine every 4 to 6 months. If the curve gets worse, your child may need to wear a brace until he or she has finished growing. In severe cases, or if bracing doesn't help, your child may need to have surgery.

Scoliosis and its treatment can be a severe strain on your child. Wearing a brace can feel and look odd. It also limits your child’s activity. Your child needs your support and understanding to get through treatments successfully.

What increases the risk of scoliosis?

Your child may be more likely to have scoliosis if someone in your family has had it and if your child is a girl. Other things that increase the chance of scoliosis include:

  • One of the bones in your child’s spine has moved forward out of place compared to the rest of the spine.
  • Your child’s arms or legs are missing or are abnormally short.
  • Your child has other problems with tissue growth that happened before birth.
